CONSENT:

By agreeing to participate in Urban-based Adventures (UBA) Telehealth Mental Health Services, you are providing your full consent, authorization and permission to receive such services from Clifton Hicks, PhD, LCSW.

POTENTIAL RISKS AND LIMITATIONS OF RECEIVING TREATMENT VIA TELEHEALTH:

Telehealth uses electronically-based platforms such as Zoom that is accessed through cell phones and computers. These platforms may not be completely secure and may potentially expose client information without notification to either client or Clifton Hicks, PhD, LCSW. I will inform clients if there is a breach of client information. Clients understands and assume these risks and limitations.

NOTICE TO CLIENTS:

The Board of Behavioral Sciences receives and responds to complaints regarding services provided within the scope of practice of licensed clinical social workers. You may contact the board online at www.bbs.ca.gov or by calling (916) 574-7830.

You are always free file a complaint with the California Board of Behavioral Science Examiners at any time. Please use my name and license number: CLIFTON HICKS, PhD, LCSW 94109. I ask that you attempt to resolve your complaint with me before filing.

PRIVACY POLICY:

Your privacy is important to me and I am committed to protecting your personal information. In this policy, I am letting you know about the ways I collect and use your information, so you can make an informed choice regarding whether you will visit Urban-based Adventures (UBA) website and enroll in Telehealth Mental Health Services.

What Information Do I Collect?

If you enroll in UBA Telehealth Mental Health Services, I will collect your name, email, address and phone number. When you submit your payment information to our payment processor PayPal and/or credit card, UBA does not have access to those numbers.

How Do I Use Your Information?

I use your name and email to send you periodic emails, including messages related to email newsletters, and occasional business announcements and updates from UBA. I will use your name and phone number to process your payment.

I use cookies (via Google Analytics) to compile aggregate data about site traffic and site interaction to optimize this website and create a better user experience for you. If you want to block cookies completely, you can disable them in your browser: Chrome, Firefox, Safari or Edge.

How Do You Unsubscribe to Emails or Subscriptions?

You can unsubscribe from receiving future emails or subscriptions at any time.

How Do I Protect Your Information?

I use various security measures to maintain the safety of your personal information, including HIPAA compliant email platforms. These services keep your information confidential, will not sell or share your information, and will not contact you directly. The third-party services we currently use are: PayPal, Squareup (merchant processing), Squarespace Campaigns (email list), Acuity Scheduling (appointment scheduling), Leadpages (website landing pages), DocuSign (contracts).

I will not trade, share, or sell your information to any other company, except that I do share your information with specific third parties who assist me in taking care of you (such as the services listed above). These third parties all agree to keep your information confidential. I will also release your information as required by law and if necessary to protect my legal rights and/or to comply with a judicial proceeding or court order, to prevent threats to others or illegal activities, or to prevent abuse of this website.

Information Regarding Children

This website is not directed to people who are under 13 years of age. We do not collect any information from anyone under 13 years of age. Please notify us at drhicks@ubatraumatherapy.com if you are a parent whose child has provided us with personally identifying information, and we will delete such information from our files.

California Residents Privacy Rights

If you are a California resident, under “Shine the Light law” (Cal Civ Code Section 1798.83) you can request the disclosure of certain personal information that we have disclosed to third parties for marketing purposes in the last year, if any. Other Legal Information

I do not collect any information from anyone under 13 years of age. This website, products and services are not directed to people who are under 13 years of age.
